As California’s largest provider of recuperative care, we bring proven expertise in safe discharges that centers dignity, compassion, and the belief that everyone deserves a safe place to heal and thrive.

As a DHCS CalAIM Community Supports provider, we serve individuals who are transitioning out of medical facilities or are at risk of hospitalization.

Our goal is to offer a safe, supportive, and structured environment with short-term housing and individualized care-including clinical and medication oversight.

Life at Soul Housing

Each one of our facilities features beds and linens, three hot and nutritious meals per day, cable TV, high-speed internet, communal areas for relaxing and socializing, ADA-accessible bathrooms and showers, complimentary laundry services, on-site clinical care, daily community programming, and even special event celebrations. Our residents are free to come and go as they please.

Eligibility for Services:

You may be eligible for Soul Housing if you are homeless, at risk of homelessness, or living in unsafe or unstable conditions. Services are also available to those recently discharged from a hospital, skilled nursing facility, or psychiatric stay, as well as individuals with medical needs who require short-term support, help with daily activities, or ongoing health monitoring.

You do not have to meet every requirement to be eligible. Instead, you only need to meet one factor in either of the following two areas:

Housing Status: This could mean you are homeless, at risk of becoming homeless, living in unsafe or n unstable housing, living alone without support, or in a home that threatens your health and safety.
Medical Condition: This could include being recently discharged from a hospital or medical facility, needing time to heal from an injury or illness, requiring help to stay medically stable and avoid readmission, needing short-term assistance with daily activities, or requiring ongoing health or behavioral monitoring.
